# How to complete and submit the CAT Exemption Application Form

### Why the form should be completed

The purpose of this document is to address the administrative process when applying for the fundamental unit standards exemption in the case where a learner is in possession of a South African matric certificate. If a matric certificate was included in the learners' POE, the assessor will assume that you are exempt from certain fundamental unit standards, and those unit standards won't be assessed. In such a case, the CAT form must be completed and signed by you, the learner.

### How to complete it

| You only need to complete the first and last page of the document, and sign where indicated ‘Applicant Signature’ on the last page. The assessor/moderator will complete the remainder according to the documentation received in the POE.
